Payment

Payment for sessions is due at date of service.  At start of treatment, you input a credit or debit card in the patient portal. I also accept Zelle, Paypal, or checks. I am happy to provide you with monthly receipts for you to submit to insurance companies for possible out-of-network reimbursement. You will be responsible for any communication with your insurance company.

Insurance

I am an in-network provider for Pacific Source and Adventist. I am happy to submit claims to these insurance providers. If you choose to use your insurance for treatment, you are agreeing to receive a psychiatric diagnosis required by  your insurance company, to maintain knowledge of your insurance information including  deductible, co-pay amounts, and to complete any phone calls  or additional research  with your insurance company needed.
